HCS & TxHmL Individualized Supports & Services
 
 
SAM's PLACE provides both TxHmL & HCS community-based services and supports to people with Intellectual and Developmental Disabilities (IDD). Individuals with a diagnosis of an intellectual disability with an IQ of 75 or below not currently enrolled in another waiver program and not exceeding specific income limits can apply for services with Texana Center, the local authority (LA). Once awarded, individuals receiving either TxHmL & HCS services may be eligible for the services and supports listed below. 
TxHmL - A DADS program providing essential supports and services to people with IDD  so that he or she may be able to continue to live with his or her family or in his or her own home. These supports and services are intended to supplement rather than replace the supports and services received in other programs or other natural supports.
HCS - A DADS program providing essential supports and services to people with IDD who live with their family, in their own home, in a host home/companion care setting, or in a residence with no more than four people who also receive services. Services meets the individual's needs so that he or she may be able to maintain himself or herself in the community.
 
Host Home/Companion Care - A HCS residential assistance service wherein the individual with IDD lives with a host companion in a home-like setting.
Supervised Living - A HCS residential assistance service wherein the Individual with IDD lives in a home-like setting with the service provider(s) present as needed for assistance.
Residential Support - A HCS residential assistance service wherein the Individual with IDD lives in a home-like setting with the service provider present 24 hrs a day (awake).
Supported Home Living - A HCS residential assistance service wherein the individual with IDD lives in his or her own home or family home with support from a service provider as needed.
 
 These supports and services are available in both programs, TxHmL & HCS.
 
Adaptive Aids - Assistance provided to individuals requiring adaptive equipment that enables the individual to retain or increase his or her ability to perform activities of daily living or to control his or her  environment.
Minor home modifications - Assistance provided to individuals requiring physical adaptations to his or her residence that is necessary to address his or her specific needs, enabling the him or her to function with greater independence or to control his or her environment.
Dental - Assistance provided to individuals requiring emergency, therapeutic, preventive, and/or orthodontice dental treatment.
Nursing - Assistance provided to individuals requiring treatment and monitoring of health care procedures that are physician prescribed or required by standards of professional practice or state law by licensed nursing personnel.
Specialized therapies - Assistance provided to individuals requiring an assessment and/or treatment by licensed or certified professionals for social work, counseling, occupational, physical, speech, audiology, dietary, and behavioral therapies.
Residential Assistance - Assistance provided to individuals including supervised living, supported home living, foster/companion care, or residential support.
Respite - Assistance provided to individuals requiring planned or emergency short term relief provided to the person's unpaid caregiver when the caregiver is temporarily unavailable. Relief is provided in the person's home or another location.
Day Habilitation - Assistance provided to individuals with acquiring, retaining, or improving self help, socialization, and adaptive skills necessary to live successfully in the community and to participate in home and community life.
Supported Employment - Assistance, supervision, and training provided to individuals who are gainfully employed to help him or her sustain that employment.
