Direct Support Provider - provides direct support and assistance to the individuals assigned to his or her care directed by each individual's IPC and any applicable state and/or facility rules, policies, and standards. Other responsibilities include providing adequate supervision based upon each individual's level of need, assisting with ADLs, transporting individuals to and from day habilitation, community activities, and/or medical and other scheduled appointments, implementing training and service objectives as well as behavioral programs, and completing all required documentation accurately and in a timely manner. Perform other duties as assigned. Varied shifts, requires some flexibility.
Preference of a high school diploma or GED and recent experience working with people with IDD and other medical and/or behavioral conditions. Requires a current and valid TDL. Must be able to perform CPR, PMAB, some heavy lifting, and the ability to perform repetitive kneeling, bending, stooping, grasping, walking, standing, sitting, and reaching above the head for long periods of time. Good written and oral communication skills.
Vocational/Habilitation Provider - provides direct support and assistance to the individuals assigned to his or her care directed by each individual's IPC and any applicable state and/or facility rules, policies, and standards. Other responsibilities include providing adequate supervision based upon each individual's level of need, assisting with vocational, etc services and activities, organizing and conducting individual and group activities, assisting with community supports and services, employment assistance and supported employment, transporting individuals to and from day habilitation, community activities, and/or medical and other scheduled appointments, implementing training and service objectives as well as behavioral programs, and completing all required documentation accurately and in a timely manner. Perform other duties as assigned. Varied shifts, requires some flexibility.
